By scheduling routine A1C tests and coordinating with your physician, we help you manage blood sugar, blood pressure, and cholesterol levels, reducing risks of heart disease and complications. Our proactive approach ensures timely adjustments to your treatment, keeping your diabetes under control and supporting your overall health.
HOW WE SUPPORT YOU
- Call patients to schedule appointments for their diabetic management testing every 3 months.
- Contact the physician's office to confirm when the last A1C test was conducted to reduce unnecessary testing.
- Call the physician's office to obtain any additional testing needed for the patient at the time of their 3-month A1C draw.
- Call patients to schedule their Annual Health Assessment, either with our provider in their home or at their PCP's office.
- Explain to patients the reason and importance of performing a yearly assessment.
- Explain to patients the importance of testing their A1C levels for diabetic management, and encourage those with levels higher than 8 to follow up with their PCP after our lab draw visit.
- Assist patients with calls to their PCP, Medical Group, and Health Plan when requested.
- Assist with Gap in Care - A1C, Colorectal Screening, Medication Review, Nephrology testing, etc. (HEDIS & CMS Star Measures).
